Put your MIOS system in "add ZWave device" mode, and then tap the tamper switch on the back of the door/window sensor to add it.

After it has been added, MIOS will be waiting for it to wake up so it can be configured.  Tap the tamper switch again to wake it up.

Note: Older version of the Door/Window sensors (up to version 3.02) did not support the wake up, which is the main reason Vera is not able to get the battery level. Starting with version 3.02 the sensors support this feature and this shouldn't be an issue.
